  • Summary of Benefits

    Group Health & Dental Insurancecontribution by MLMH & Employee 

    Participation will be available to employees working 24 or more hours per week. Option to include family members with employee picking up premium difference. Premium dollars are non-taxable. 

    Group Life Insurance — contribution by Employee 

    Opportunity to purchase life insurance for you and your dependents at reduced rates when employed 24 hours or more per week. 

    Flex Spending Accounts — contribution by Employee 

    Pre-tax dollars used to pay eligible healthcare and dependent care costs. 

    Personal Medical Disability Insurance — contribution by MLMH 

    Pays 65% of your base pay rate during periods of medical disability extending beyond 30 calendar days for employees who work 36 hours or more a week. 

    Retirement Plan — contribution by MLMH & Employee 

    401(k). The Employee may choose to contribute from 1% to federal caps each pay period. MLMH matches 50% up to 6% of employee's election. In addition, an annual year-end discretionary contribution of 2% may be made. 100% vested after three years. 

    Tax Sheltered Annuity — contribution by Employee 

    You choose from selected vendors. Tax shelter a portion of your income and save for your personal retirement. 

    Paid Time Off (PTO) — contribution by MLMH 

    PTO accrues from day one, combines traditional holidays, vacations and sick time into one account. It is accrued bi-weekly at a rate that is based on length of service, job classification, and number of hours worked. Maximum accrual levels established for each of the three rate levels. 

    Holiday Premium Pay — contribution by MLMH 

    Time and half for working major holidays: New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and Christmas Day. 

    Bereavement Leave — contribution by MLMH 

    Leave to attend or make arrangements for a funeral of a family member. 

    Educational Assistance — contribution by MLMH 

    If criteria is met, reimbursement at the current approved rate for eligible job related classes leading to a degree, national certification, job enhancement courses and some required in-services for accreditation are paid. 

    Excel Program — contribution by MLMH & Employee 

    Fitness center available during scheduled hours throughout the day and evening. Small annual membership fee. 

    U.S. Savings Bonds — contribution by Employee 

    Purchase U.S. Savings Bonds through automatic payroll deduction. 

    Pharmacy — contribution by MLMH & Employee 

    Employees are entitled to have their prescriptions filled at current employee discount rate. Limited supply of cold/flu medicines and pain remedies available. 

    Cafeteria — contribution by MLMH & Employee 

    Employees may obtain a Cafeteria discount by wearing their employee name badge.

    Supportive Therapy Services will be provided for personal, family or marital problems at a reduced rate.

    Funds available for employees experiencing financial hardship/catastrophes. 

    Teddy Bear Program — contribution by Employee 

    Child care provided through the Pediatrics Unit to children who are not able to attend normal child care due to illness/injury, pending space available. 

    Work Break — contribution by MLMH 

    Up to fifteen minutes may be taken for each four hours worked at the discretion of the Supervisor. 

    Employee Health Services — contribution by MLMH 

    Full time on-site employee health nurse available. Flu shots, rubella titer, TB test, Hepatities B antibodies offered at no cost to employees. 

    CPR Classes — contribution by Employee 

    Classes are provided to MLMH employees at nominal charge. 

    Employee Recognition — contribution by MLMH 

    Employee Service Awards Banquet and Employee of the Quarter Program throughout the year recognize employees. 

    Social Events — contribution by MLMH 

    Various hospital sponsored activities offered throughout the year, some include family members. Included in these activities are the hospital cookout, employee family picnic and annual Christmas dinner. 

    Jury Duty — contribution by MLMH 

    MLMH will pay the difference between the amount employees would have earned from their normal work schedule and the amount they are compensated by the court. 

    Social Security — contribution by MLMH & Employee 

    Income after retirement and in case of disability

    Protection under Nebraska Worker's Compensation Act. Medical and income for employees who suffer a job related injury. 

    Leave of Absence — contribution by MLMH 

    A leave may be granted for personal, medical, educational and military absences.  Subject to approval. 

    Family & Medical Leave (FMLA) — contribution by MLMH 

    A leave will be granted for up to 12 weeks, with or without pay, to employees with family or medical needs, after one year employment and 1,250 hours worked in the 12 months prior to leave. 

    Liability Coverage — contribution by MLMH 

    While functioning in your position as an MLMH employee, you are insured under the MLMH general & professional liability plan. 

    Intervent Lifestyle Management Program — contribution by Employee 

    Personalized wellness mentoring program. Participants learn skills in areas of nutrition, stress management, exercise, weight management and tobacco cessation helping to promote healthier lifestyles. Includes blood work analysis, blood pressure, one to one mentoring and educational kits. Payroll deduction option. Spouses are also eligible for this program. Employees who complete first year of program are eligible for a program rebate.
